There is definitely a bulge there, and our friend Chevalier here has an ambiguous gender, so this is either otoko no ko or futanari. Not gonna change the tags because I'm not sure which (the artist seems to be more likely to draw otoko no ko, but I think I see a bit of a bust there).

Chevalier d'Eon is a genderfluid trans woman in Fate/GO. It matches her real life identity as she had her legal gender changed in 18th century France by convincing the king she was a woman. So no, it's not otokonoko nor futanari. Astolfo is an otokonoko, d'Eon can be either a man or a woman but was born with a penis despite identifying as a woman for the latter half of her life. She's actually a very fascinating historical figure and Fate/GO have done a very good job representing her, I highly recommend reading about her.

So depending on how one's knowledge about her she can be seen either way.

Oh, really? I'd read that her gender in-game was listed as "?" or something. The more you know. EDIT: To clarify a bit more, I'm a transwoman myself, it just never occurred to me until that post was made that Fate would be inclusive in that regard.
What you can visually confirm should take priority over outside knowledge for tagging purposes right? You won't tag a genderbent character it's original gender because the lore says so, for instance. Also, many fate characters have a swapped gender. As far as I know, the fate's d'Eon, the way it was manifested by the Grail, can change his/her gender with the Self-Suggestion skill. The bulge implies his "current" gender is male. Actually, even if the picture won't be tagged 1boy because "can be a futanari" or "is self identified as a woman", I think the bulge tag shouldn't be omitted.. It doesn't implies any gender tag anyway.
Tagging what you see has to take exceptions, specially when crossdressers literally look like a different gender and get tagged based on their information (astolfo is a great example) Unless d'eon is drawn with make or female parts showing, it can't be based on what you see because logical reasons stated
