The tags loli and shotacon are hidden for the sake of the advertisers, so they don't appear on pages viewed by regular member accounts that are ad-supported. It would make sense to add this tag to that list of tags to be hidden though.
As for use, I'd say it applies wherever creepy old men are featured in an appropriate context. It doesn't seem to be applied particularly uniformly though. Most citations seem to be related to textual references, which is pretty much counter to our "tag only what you see" policy.

There are a number of tags that are only applied when there's appropriate text in an image. It's still "tag what you see" because you see the text.
