Basically, a combo is being broken. There are some things that match, and something disrupts that, like 11121. I belive the phrase originated from a fighting game.
I think there's a knowyourmeme page explaining it.
It's a bad name. Better would be "odd one out" or something like that... and it has only a single image.

I totally agree with S1eth . If nobody suggests a better name, I think I could simply rename the pool to "odd one out".
Or "one of these things is not like the others"
I've renamed it to "Odd One Out" for brevity.
