Breasts too small don't have "a noticeable upward 'perk.'"
Of the 4 example posts above, only #3 has the tag properly applied. #1 is arched back. #2 actually sags downward as best as breast so small can, and #4 doesn't have enough to have a perk.
#3's breasts are arguably too large for flat chest, though.
