I've seen many touhou doujin, from hentai,cute,guro,or strage one. But this one is just too damn weird for having many thing mixed up....and I don't understand the story, translation please.

From what I've gathered, it was supposed to be a scary way of seeing the events of The Embodiment of Scarlet Devil, but Rinnosuke dies while in the middle of telling the story and...

EoSD. Reimu is the youkai-killing dictator of Gensokyo. During the "Scarlet Mist Incident", Reimu came to exterminate the source of the red mist that suddenly appeared throughout Gensokyo. Remilia, who only wanted to be able to go out during the day at her leisure, was about to be killed by Reimu. Then, the narrator, Rinnosuke, suffered a fatal heart attack and the wicked Reimu was no more. The quest for the precious thing could continue.

Kudos to the artist for being able to draw something cute and creepy at the same time.

And by something cute, I mean Remilia.
