I never actually said "do not delete", though it is implied. However, I'm not sure I understand how you mean it's shallow.

It's a retort that I'd expect from a child. I just can't take it seriously. It's like a child (we'll call him John) who got caught in the act of pushing down a younger kid. When scolded by an adult, John's friend argues, "but Mark pushes kids more than him!" in an attempt to get John out of trouble. It doesn't change the fact that John pushed a little kid to begin with.
I expect you're referring to guro when you say "worse than this." That only goes over the content-matter. The other reason this was flagged was because it is badly drawn. Badly drawn guro also gets deleted.. and some well drawn guro, as well. The only reason you don't have so many grotesquely fat pictures here is because that's not nearly as popular or common as guro. I don't think it's wise to make an argument when almost the entirety of the audience disagrees with what you're saying (at least for something as unimportant as this)
OK. More reasonable points supporting this could then be that the site is for fanart pieces as long as they are not in direct violation of site rules. If this doesn't violate a rule then it can be said it was only flagged because someone just doesn't like it.
Secondly: the moderators do seem to have a bit of a twisted sense of humor. They have approved pictures just to make the regular viewers panic.
While you may find my reason childish it is somewhat justified in this case. This isn't the only fat/obese picture on Danbooru, so it is strange to to target just this one.
Or were you just looking for an argument? Well if you were then you win, I concede.

Except that this does. It's directly stated in the TOS:
Grotesque: Any depiction of extreme mutilation, extreme bodily distension, feces, or bodies that are far outside the realm of normal human proportion (for example, breasts or penises that are larger than two heads in size).
Guro is borderline on this list, but this image qualifies not only because of the huge breasts, but because of the absurd amount of bodily distension portrayed here.
