Don't flag duplicates unless you parent them to the canonical copy. Otherwise it's impossible to check whether things actually are duplicated. Just because you know something doesn't yet help anyone, you have to communicate that appropriately.

At the end of Part 9 Koishi's amputated and decapitated corpse sprouted tentacles and "walked" over to Yukari's bullet and explosion-riddled corpse and combined with it somehow. Most of that isn't actually shown. At the end you see a blond-haired Koishi.
The tentacles have something to do with her third eye, but I'm a bit confused on that one.

And she wasn't killed by Reisen. Because she has the power to manipulate the border between life and death.
She was so crushed by the death of her family that she lets herself die, but her death was stopped by Koishi. (because if she really died... well that would be bad for everyone in the story but Koakuma)

That is incredibly STUPID! I mean even for that series
