The best sense I've been able to make of it, thanks to some Google+ discussion:

Ralph, it seems, was Trevor Belmont's name in the Japanese version of Castlevania III. And since Sayaka's sporting a rather impressive fang, I'm guessing that's where the artist was going with this one.
