When Metal Gear Online 2 was still running, there was a mask ingame that player characters could wear, resembling the head of Robbie the Rabbit from Silent Hill.
I'm still surprised that they didn't show Old Snake confronting his personal demons in MGS4 like Big Boss did in MGS3. I can think of a few ways MGS4 could have incorporated Silent Hill-like elements without drastically altering the story.
Also, as of this writing, Hideo Kojima (creator of Metal Gear Solid) has been asked to direct the 9th Silent Hill title. The announcement of that news could have been the inspiration behind this picture.
