But he is pointing to something behind him with his right arm, and resting the left one on the throne. That's impossible unless his arm is on backwards (he literally has two left arms), and looks really awkward. Which is a shame, because the rest of the image is great.
I think you're seeing Doom as being turned towards the viewer, instead of mostly parallel to chrisg? Or the rock he's leaning his arm on.
Even if he's turned towards us, his arm is still somehow twisted so it points directly behind him. And his hand is somehow upside down. The more I look at it, the less I understand what's going on.

No I mean, he's not turned towards us, but a bit away - it's just since his shoulder is obscured by the cloak it's hard to tell.
