Well, the guidelines say that violence does not affect the Rating.
It's only an indicator for sexual content.

According to the rating guidelines:
Rating:Explicit: For pornography — images containing explicit sex, gratuitously exposed genitals, or that are otherwise pornographic.

Ratings only apply to sexual content. Violence, gore, or profanity do not affect an image's rating.
