Context; when you're alone in some place remote there's no one else around. While if you're in a city there are constantly people around you to reinforce your loneliness and make you think about it.
No different than anything else really. Which would be worse after all; being hungry while being just out and about some place, or being hungry while food you can't eat is sitting right in front of you? Seeing that which you don't think you can have amplifies pain and anguish a great deal.
