Probably just like maids in other contexts do, only that all of them are married people.
It's not shown so far how in general things work, but it seems some people still continue to get married in a way analogous to the normal way. The main difference is I guess the husband-to-be doesn't really know how his wife-to-be will look beforehand (but you can reverse the process though I don't know how likely that is)
No, I'm pretty sure meido in animu are incapable of being in relationships with anyone but their masters or possibly other servants of the same master. The prevalence of maids serving mistresses (and hence, being incapable of breeding) directly leading to their rarity to the point of mythicalness, and hence Sakuya being a part of Gensokyo.

Incidentally, I just realized that this world would lack one of the social repercussions of a society accepting of harems (i.e. Saudi Arabia), such as a gender imbalance caused by the upper classes taking all the women, as women are always available so long as there are people.
However, this may also mean there's few cross-class marriages or even more inbreeding akin to the European dynasties of the past (assuming the male and female bodies share genes). For regular people, at least.

That gender ratio thing was exactly what I was trying to bring up with the wolfmen and goblins, though... It's exactly whatever people are going to stop at for whatever reasons they face. (With, presumably, the poor having smaller families because of the limits of what they can feed and house.)
I don't see why gender swapping would alter the chances of interbreeding, unless the male/female ratio became SO skewed through giant harems that there started to be only a few bloodlines left.
And cross-class marriages generally were common and accepted in polygamous cultures. At most, only the "first wife" would be of a noble family and confer status or family alliance. All the rest were just for procreation, so if they had a working ovary, they would do.
