I think she's tired or under the influence of something and the Admiral caught her before she folded over.

I think the admiral is choking her to unconsciousness so she can "rest"...

Edit: According to the artist's tweet, he intended to draw the admiral who managed to catch Etorofu right before she collapse from fatigue, but somehow the hand was drawn at a very misleading position.
