Most of her crystalization is internal, but she should definitely be showing ore lesions on her wrist.

Not even official art shows any ore lesions on her skin, only her base form art has some visible infection lines on her right wrist, no other portrait or skin shows any sign of infection. The infection progresses differently for everyone, so any crystal formation could be exclusively internal, but that wouldn't really explain people being able to tell she's infected just by looking at her.

Well she is the leader of Rhodes and it seems to be a well known entity in their world. It might just be common knowledge or assumed she's infected.
