Added bad_anatomy tag. No matter how flexible you are, even with dislocation involved, there's no way you can make your hands meet with thumb on that position.
This hand/arm position is called "reverse prayer" and is even used in Yoga. Google it and you'll find lots of videos and pictures with thumbs together and without dislocation. I'm not implying that it's easy to do, but tagging bad_anatomy is just wrong here.

I do know that pose, and can do it rather easily. The problem with this picture is the height and position of the clasped hand; the higher you go, the more metacarpals below your pinkie are pressed together, and that prevents you from making your thumbs meet. Not to mention, your hand would also be obstructed from going higher by your forearms. In the picture whoever tied the girl forced the thumbs together with rope, and even with foreshortening taken in account, there's no way she can reach up to just below the neck unless she dislocated her shoulders.
