Actually, this game is animated. So the one with smaller looking breasts is just part of the frame.

I could be wrong but the way they animated this game is by distorting still image, so the frames may look pretty stretchy and distorted.

Wasn't there a Boing series that lets you choose the size of the breast of characters? I think this might be it, but can't recall clearly.

KJlost is right. The CG packs found around the internet have all the scenes with two different breast-size options.
