It's an important distinction. Though, it begs the question: if someone had AIDS or something and somehow got cured, would you feel ENTIRELY comfortable with having sex with them, even if it was pretty much a safe bet?

I don't see the comparison here. Yeah, Sakura's childhood was seriously fucked-up, but I don't see why that means she can't have sex when in a loving relationship with Shirou. He's going to have to be careful, I suppose, that he doesn't do anything to damage her mind even more, but judging by HF True (in comparison to HF Normal), he seems to have done a pretty good job of fixing it just by being there.
