from what i see, that girl is sitting on a weird ass chair, with her legs spread. She's also wearing a hige shirt, it seems.

I decided to look around the gyn_chair tag, then I see this one post with a weird yet doable design. Doable, as in I can imagine finding these kind of things if anyone would ever make one. I can't really tell what those levers do or those other bits go and that's interesting.
