The OP said that this picture could get you in trouble because it's too realistic. That is not true at all. Obscenity is the determining factor; realism is irrelevant. As to whether this image would be considered obscene, I have no idea. That's something that's decided on a case-by-case basis by a jury.

No, this law still stands. The Supreme Court ruled another act overbroad in 2002 because it forbade all sexually-explicit depictions of minors. In response Congress passed this law, which only bans obscene depictions of minors.
If its just a nude child on its own then most people won't cry "CP!". Pictures on how to powder your baby's ass won't get you in jail. You usually have to have the child depicted in an act of some kind (generally lascivious in nature) with another individual. And kids like to tear their clothes off and run around the streets buck naked, much to the chagrin of their parents. Point being, not everyone is terrified of their own and other people's bodies. True, but no case invoking this law has been before the Supreme Court, which has struck down similar laws as unconstitutional in the past. Also, to the best of my knowledge, everyone convicted under this law also possessed actual child pornography along with cartoons (not that that makes the law a good one, but hey, they were going to jail for a long time anyway).
Of course, all that means is that your fate is in the hands of lawyers if you're busted, but on the plus side, the ACLU would probably spot you a good one.
