It's a girl with a penis. The artist stating she's transgender is more or less confirmation that she doesn't have a vagina.
To play the devil's advocate, there are in fact transgender surgeries that create a vagina while leaving the penis intact, or vice versa, so having one doesn't necessarily preclude having the other. I'm not saying the tags should be changed, but it's naive to assume that all transgender people fall into one of two particular genital configurations.

This is true, but obscure enough that it doesn't really register in the world of anime fanart (and that's saying something). The average person drawing a trans girl with a penis is implying that she's pre-op and has only one set of genitals, which is something that was getting lost in this discussion.
